بررسی علل اشتباهات دارویی و عوامل مؤثر بر عدم گزارش‌دهی آن‌ها از دیدگاه کارکنان پرستاری بیمارستان‌های وابسته به دانشکده علوم پزشکی نیشابور |

مقدمه: اجرای دستورات دارویی بخش مهمی از فرایند درمان و مراقبت از بیمار می‌باشد. گزارش‌دهی اشتباهات دارویی از سوی پرستاران موجب حفظ ایمنی بیمار و عدم گزارش‌دهی مناسب می‌تواند باعث ایجاد مشکلات جدی در سیستم‌های سلامت شود. هدف از این مطالعه، تعیین علل اشتباهات دارویی و عوامل مؤثر بر عدم گزارش‌دهی آن‌ها از دیدگاه پرستاران بود. روش: مطالعه حاضر از نوع توصیفی- تحلیلی بود و جامعه آن را 248 پرستار از بیمارستان‌های وابسته به دانشکده علوم پزشکی نیشابور تشکیل می‌داد که به وسیله سرشماری انتخاب شدند. جمع‌آوری داده‌ها به وسیله پرسش‌نامه بود. تجزیه و تحلیل داده‌ها با استفاده از آمارهای توصیفی و استنباطی (آزمون t) با استفاده از نرم‌افزار SPSS نسخه 16 صورت گرفت. یافته‌ها: کمبود تعداد پرستار (2/1 ± 3/4)، خستگی ناشی از کار اضافی (05/1 ± 1/4) و تراکم زیاد کارها (8/2 ± 1/4) از مهم‌ترین علل اشتباهات دارویی بود. همچنین مهم‌ترین علل عدم گزارش‌دهی اشتباهات دارویی، تمرکز مسؤولان تنها بر شخص اشتباه کننده بدون توجه به سایر عوامل دخیل در بروز اشتباه (06/1 ± 86/3)، ترس از مسایل قضایی (07/1 ± 79/3) و واضح نبودن تعریف اشتباه دارویی (13/1 ± 34/3) بود. بین عوامل مؤثر بر بروز اشتباهات دارویی از دیدگاه کارکنان با نوبت کاری ثابت و در گردش اختلاف معنی‌داری وجود داشت (05/0 > P). نتیجه‌گیری: با توجه به اهمیت و تأثیر انکارناپذیر بحث ایمنی بیماران، لزوم ایجاد یک سیستم گزارش‌دهی و ثبت خطا به همراه واکنش مثبت مدیران در قبال بروز اشتباهات کارکنان ضروری می‌باشد. |

Background & Aims: Administration of medications is an important part of treatment. Reporting of medication errors by nurses maintains patient safety and the lack of appropriate reporting can cause serious problems in health systems. The aim of this study was to determine the causes of medication errors and the barriers of error reporting from the viewpoints of nurses. Methods: This cross-sectional study was conducted on 248 nurses in hospitals affiliated to Neyshabur University of Medical Sciences, Iran. Participants were selected by simple random sampling. Data were collected through a questionnaire and analyzed by descriptive and inferential statistics in SPSS software. Results: The most important reasons of medication errors were nursing staff shortage (4.3 ± 1.2), fatigue due to overwork (4.1 ± 1.05), and high workload (4.1 ± 2.8). The main reasons for not reporting medication errors were authorities' focusing on the person who has made the error regardless of other factors involved (3.86 ± 1.06), fear of legal issues (3.79 ± 1.07), and lack of clarity of the definition of medication error (3.34 ± 1.13). There was a significant difference between the factors affecting medication errors from the view of nurses, and fixed and rotating work shifts (P < 0.05). Conclusion: Due to the importance of patient safety, establishing a system for reporting and recording errors along with the positive reaction of managers to errors by personnel is essential. |
