عنوان فارسی بررسی ارتباط عوامل انسانی تأثیرگذار در حوادث معدن با استفاده از رویکرد سیستمی
چکیده فارسی مقاله سابقه و هدف: بررسی سیستماتیک حوادث در سیستم‌های فنی ـ اجتماعی مانند صنعت معدن در کاهش حوادث اهمیت دارد. در این مطالعه قصد داریم ارتباط میان عوامل انسانی مشارکت‌کننده در حوادث معدن را با رویکرد سیستماتیک بررسی کنیم. مواد و روش‌‌ها: به‌منظور انجام این پژوهش، حوادث رخ‌داده در یک مجتمع بزرگ معدن سنگ آهن واقع در جنوب کشور در یک دورۀ 7 ساله جمع‌آوری و سپس مهم‌ترین حوادث غربال شد. با کمک تیم تحلیل حادثه و بر اساس چارچوب تجزیه‌وتحلیل عوامل انسانی و طبقه‌بندی آن، عامل هر حادثه شناسایی و طبقه‌بندی شد. در مجموع داده‌های مربوط به 154 حادثه جمع‌آوری شد. در ادامه، داده‌ها با استفاده از رگرسیون لجستیک در نرم‌افزار SPSS نسخه 16 تحلیل شد. یافته‌ها: خطاهای مبتنی بر مهارت بیشترین فراوانی را در بین اعمال ناایمن به خود اختصاص داد. نواقص محیط فیزیکی، طراحی نامناسب عملیات و فرایندهای سازمانی بیشترین فراوانی شناسایی در عوامل سازمانی حوادث منتخب بودند. در کل سیستم، وجود برخی نواقص در سطوح بالایی، نقص در سطح پایین‌تر را پیش‌بینی می‌کرد که شامل جوّ سازمانی و نظارت ناکافی، فرایندهای سازمانی و طراحی نامناسب عملیات، نظارت ناکافی و محیط فیزیکی، طراحی نامناسب عملیات و محیط فیزیکی بود. نتیجه‌گیری: خطاهای مبتنی بر مهارت، مشکلات نظارتی و فرایندهای سازمانی جزء نواقص با فراوانی زیاد در ایجاد حوادث در صنعت معدن هستند. تحلیل ارتباط میان فاکتورهای علیتی در سطوح مختلف در اتخاذ استراتژی‌های مناسب برای کاهش حوادث اهمیت دارد.

عنوان انگلیسی Investigation of the Relationship among Human Factors in Mining Accidents Using a Systematic Approach
چکیده انگلیسی مقاله Background and Objective: Systematic analysis of accidents in sociotechnical systems, such as mining industry, is very important to reduce accidents. The present study aimed to analyze the relationship among human factors in mining accidents using a systematic approach. Materials and Methods: The accidents occurred during seven years in a large iron ore mine complex in the south of Iran were collected, and the most important events were selected subsequently. Afterward, all contributing factors in each accident were identified using an accident analysis team by Human Factors Analysis and Classification System (HFACS) framework. A total of 154 accidents were collected, and the data were analyzed in SPSS Software (version 16) using logistic regression. Results: Skill-based errors were the most frequent among the unsafe acts. Furthermore, the deficiencies in physical environment, as well as inappropriate operations and organizational procedures were the most frequently identified deficiency in the selected accidents. Throughout the overall system, the existence of some deficiencies in upper levels predicted those in lower levels, which included organizational climate, inadequate supervision, organizational procedures and inappropriate operations, inadequate supervision and physical environment, as well as inappropriate operations and physical environment. Conclusion: Skill-based errors, supervision problems, and organizational procedures are the most common deficiencies in mining industry accidents. Analysis of the relationship among the causal factors at different levels of HFACS is necessary to adopt appropriate strategies to reduce the accidents.
