عوامل مرتبط با کیفیت اخذ رضایت آگاهانه در بیماران بستری تحت عمل جراحی، یک مطالعه تحقیقی در بیمارستانهای شهر کاشان در سال ۱۳۸۶
چکیده فارسی مقاله
مقدمه: رضایت آگاهانه پایه و اساس اخلاق پزشکی است. این پژوهش با هدف تعیین عوامل مرتبط با کیفیت اخذ رضایت آگاهانه در بیماران تحت عمل جراحی در بیمارستانهای آموزشی کاشان انجام گردید. روشکـار: در این پژوهش مقطعی 300 بیمار (رضایتدهنده) بخش جراحی بیمارستانهای آموزشی کاشان در سال 1386 به صورت تصادفی ساده با توجه به برنامه زمانی پذیرش وارد مطالعه شدند. دادهها از طریق مصاحبه با بیماران بعد از پذیرش و قبل از جراحی با استقاده از پرسشنامه، جمعآوری و با آمار توصیفی و تحلیلی، تحلیل شد. یافتهها: وضعیت ارایه اطلاعات، قابل درک بودن فرم رضایتنامه، داوطلبانه بودن و رابطه پزشک با بیمار به ترتیب در حد متوسط، ضعیف، ضعیف و عالی بود. در محور ارایه اطلاعات، ضعیفترین وضعیت مربوط به بیمارستان عمومی و جراحیهای عمومی بود (001/0 p < ). کسانی که از بروشور استفاده کرده بودند فرم را بیشتر درک می نمودند. 7/88% اظهار کردند که پزشک یا جراح میبایست عوارض شدید (حتی مرگ) را توضیح دهد. 7/56% از کسانی که معتقد به بیان عوارض بودند، عنوان نمودند که عوارض به آنها گفته نشده است. همچنین ارایه اطلاعات بیشتر با کاهش نگرانی همراه بود(05/0 p < ). نتیجهگیری: وضعیت اخذ رضایتنامه مناسب نیست و بیماران مایل به دریافت اطلاعات بیشتر (حتی عوارض شدید) هستند. معرفی سایر منابع اطلاعاتی (از جمله ارایه بروشور) بر سه محور داوطلبانه دانستن اخذ رضایتنامه، درک فرم رضایتنامه و ارتباط بین بیمار و پزشک تأثیر مثبتی دارد و میتواند به عنوان یک راهکار مناسب مد نظر قرار گیرد. نهایتاً به نظر میرسد ارایه اطلاعات کامل و متعادل ضمن اینکه موجب افزایش آگاهی بیماران میشود، تأثیر سوء بر نگرانی بیمار نمیگذارد.

Quality of Informed Consent Process and Factors Affecting it among Patients Undergoing Surgery, an Empirical Study in Hospitals of Kashan, Iran

Iran Received: 15 Oct 2009, Accepted: 9 Jan 2010 Abstract Introduction: We studied the factors that affect quality of informed consent process among patients undergoing surgery in the teaching hospitals in Kashan province, Iran. Methods: In a cross sectional study, we surveyed 300 randomly selected patients in teaching hospitals of Kashan, Iran in 2008 and interviewed them before surgery, using a questionnaire. Patients were asked about disclosure of information, voluntary decisions, consent form understandability, and physician-patient relationship. We used descriptive and analytical approaches to analyze our data. Results: Consent-givers evaluated the information disclosure, comprehensibility, voluntariness and interaction as “average”, “poor”, “poor” and “excellent”, respectively. Information disclosure was worse in the general hospital and general operations (p< 0.001). Those who had received information brochures reported higher levels of comprehensibility for the forms. 266 patients (88.7%) stated that the physician or surgeon must explain the severe complications including risk of mortality (P < 0.0001) however, 56.7% had not been informed about the complications. In addition, learning the information was associated with lower level of anxiety (P< 0.05). Conclusion: The present i nformed consent process seems not to be of high-quality. Patients like to receive more information even about severe complications or risk of mortality. Therefore, appropriate approaches to enhance patients’ awareness, especially in general hospitals, seems to be necessary. Providing alternative information, including brochures, have positive impacts on giving voluntary informed consent, comprehensibility of the consent form and the physician-patient relationship. Finally, conveying appropriate information will not lead to anxiety among patients. Key words: Informed Consent, Inpatient, General Surgery.
