عنوان فارسی عوامل مؤثر بر واحد ارزش نسبی خدمت ویزیت پزشک عمومی شاغل در مطب در شهر تهران
چکیده فارسی مقاله چکیده مقدمه: ارزش‌گذاری واقعی و منطقی خدمات بهداشتی و درمانی همواره به عنوان یکی از مهم‌ترین عوامل تأثیرگذار بر اجزای نظام سلامت در مبحث اقتصاد و درمان مطرح بوده است. از این رو پژوهش حاضر با هدف بررسی تأثیرعوامل مختلف بر واحد ارزش نسبی ( RVU ) خدمت ویزیت پزشکان عمومی در مطب در شهر تهران صورت پذیرفته است. روش کـار: پژوهش حاضر یک مطالعه تحلیلی می‌باشد که طی آن دیدگاه سه گروه مشتمل بر پزشکان عمومی (668 نفر)، مراجعین به مطب‌های پزشکان عمومی (583 نفر) و کارشناسان (22 نفر) در خصوص تأثیر عوامل مختلف بر ارزش ویزیت پزشکان عمومی در مطب شهر تهران از طریق پرسشنامه جمع‌آوری و با استفاده از آزمون‌های آمار توصیفی و ضریب همبستگی پیرسون مورد تجزیه و تحلیل قرار گرفتند. یافته‌ها: در پژوهش حاضر در رابطه با عوامل مؤثر بر تعیین ارزش نسبی ویزیت پزشکان عمومی، مهارت پزشک از نظر کارشناسان، شدت بیماری از نظر مراجعین، مدت زمان ویزیت از نظر پزشکان، خطای پزشکی از نظر مراجعین، خطر برای پزشک از نظر کارشناسان و ویزیت پزشکان از دیدگاه کارشناسان بیشترین امتیاز را داشت. بین دو متغیر مدت زمان ویزیت و مهارت پزشکی از دیدگاه پزشکان ارتباط معناداری مشاهده نشد (01/0-= r و 72/0= p ). بین مهارت پزشک و خطای پزشکی نیز ارتباط معنادار نبود (01/0= r و 78/0= p ). نتیجه‌گیری: نتایج پژوهش حاضر می‌تواند راهنمایی برای تعیین ارزش واحد نسبی سایر خدمات پزشکی باشد و مدیران در سطوح مختلف وزارت بهداشت، سازمان مدیریت و برنامه‌ریزی و سایر سازمان‌های مربوطه می‌توانند در تعیین تعرفه‌های واقعی مطابق با شرایط اقتصادی و محیطی کشورمان با توجه به نتایج پژوهش حاضر برنامه‌ریزی به عمل آورند.

This study aimed to define the effective different factors on relative value units (RVU) of GP’s visit in private offices in Tehran. Methods: This is an analytical study. The views of three groups including 668 general practitioners, 583 patients referred to general practitioners, and 22 national experts regarding the factors that affect the relative value unit of general practitioners visit in their private offices in Tehran were gathered using a self-structured questionnaire. Data were analyzed using descriptive statistical methods and Pearson correlation coefficient. Results: Regarding factors influencing relative value unit of GPs' visits ، physicians' skills, severity of illness, duration of visits, medical errors, risk for physicians, and physicians' visits were most influential. There was no significant relationship between the duration of visits and the physicians' skills from the physicians point of view (r= -0.01, p= 0.72). There was also no significant relationship between physicians' skills and medical errors (r= 0.01, p= 0.78). C onclusion: The findings can help determining the relative value units applied to other medical services and management levels. The Ministry of Health, Management and Planning Organizations and other relevant organizations can use the results of this study to determine the real costs.
