عنوان فارسی مطالعه چالش‌های اجرای پزشک خانواده در نظام سلامت ایران
چکیده فارسی مقاله دریافت: 24/5/94 پذیرش: 17/9/94 مقدمه: یکی از مهم‌ترین سیاست‌های مطرح شده در دهه اخیر در عرصه نظام سلامت‌‌، برنامه پزشک خانواده است که بر طبق مواد قانونی موجود تاکنون باید به صورت ملی اجرا می‌شد‌‌، ولی اجرای آن محدود به روستا‌ها و به صورت پایلوت در دو استان فارس و مازندران شد. از این رو پژوهش حاضر با هدف مطالعه فهم بهتر چالش‌هایی اجرایی پزشک خانواده بر اساس نظریه تضاد صورت گرفت. روش کـار: این مطالعه کیفی مبتنی بر نمونه‌گیری هدفمند انجام شد. تکنیک مصاحبه و تحلیل اسنادی جهت تولید داده‌ها‌، و تحلیل محتوی جهت تحلیل داده‌ها به کار گرفته شد. در مجموع، 26 مصاحبه با سیاست‌گذاران‌‌، مجریان در سطوح ارشد کشوری و صاحب‌نظران مطرح و کلیدی در زمینه پزشک خانواده صورت گرفت. یافته‌ها: چالش‌های اجرای پزشک خانواده تحت پنج مقوله اصلی شناسایی شدند‌‌، که عبارتند از: «چالش ناشی از ساختار حاکم بر نظام سلامت»‌‌، «ماهیت چالش‌ برانگیز پزشک خانواده»‌‌، «پزشکی کردن مدیریت»‌‌، «تضاد منافع»‌‌، «روابط حاکم بر وزارتخانه‌ها و سازمان‌های دخیل دربرنامه پزشک خانواده». نتیجه‌گیری: چالش‌های اجرای پزشک خانواده در شرایط تنش‌، تضاد و ناهماهنگی بین اجزای مختلف نظام سلامت در ابعاد مختلف، بهتر فهمیده می‌شوند. در واقع از میان سه نظریه مطرح در جامعه‌شناسی پزشکی، چالش‌های پزشک خانواده در قالب نظریه تضاد بهتر قابل فهم بوده، هر چند که گاه برخی از یافته‌‌ها فراتر از نظریه تضاد هستند.

Research Center of Regional Office of World Health Organization Original Article Received: 15 Aug 2015, Accepted: 8 Dec 2015 Abstract Introduction: Family physician project is one of the most significant policies in health system during the recent decade which was already supposed to be applied nationally legally, however its implementation has been limited rural areas and two provinces of Fars and Mazandaran, as pilot locations. This study was conducted to achieve a better understanding of the challenges of family physician projects implementation using the conflict theory. Methods: The qualitative study was conducted using convenience sampling method. Data was collected using interviews and documents analysis techniques, and was analyzed using content analysis. Total number of 26 interviews were conducted with national politicians and high level executives, and key informants in family physician project. Results: Family physician implementation challenges are identified in five main categories: challenge caused by the governing structure which affects health system, challenging nature of family physician, medicalization of management, conflict of interests, relationships among related ministries and organizations involved in family physician project. Conclusion: A better understanding of different aspects of the family physician implementation challenges would be achieved under magnificent conflicts, tension and inconsistency among health system components. The family physician project challenges would be explained better using conflicts theory compared to the other significant medical sociology theories; however some data was beyond the conflicts theory.
