عنوان فارسی اثرات رفاهی و اقتصادی افزایش مخارج بهداشتی و درمانی دولت در ایران (رویکرد ماتریس حسابداری اجتماعی)
چکیده فارسی مقاله دریافت: 97/4/30   پذیرش: 97/7/16 چکیده مقدمه: بخش سلامت از بخش‌های اساسی جامعه بوده، ارتباط متقابلی با سایر بخش‌های اقتصاد دارد. با توجه به ضرورت دخالت دولت در بخش سلامت ایران و اهمیت شناخت اثرات آن، تحقیق حاضر به بررسی پیامدهای اقتصادی و رفاهی افزایش مخارج بهداشتی-درمانی دولت در ایران پرداخت. روش کار: در مطالعه‌ی کاربردی حاضر، از ماتریس حسابداری اجتماعی سال 1390 مرکز پژوهش‌های مجلس شورای اسلامی استفاده شد، که با استفاده از آمارهایی از قبیل حساب‌های ملی، آمارگیری از بودجه خانوار سال 1390 و به‌هنگام‌سازی جدول داده-ستاده سال 1380 تهیه گردیده است. برآورد نتایج و تحلیل داده‌ها با استفاده از نرم‌افزارExcel 2013 انجام شد. یافته‌ها: نتایج نشان داد که 99درصد تقاضا برای بخش بهداشتی- درمانی دولت به‌صورت تقاضای نهایی بوده و 67درصد پرداخت‌های این بخش برای جبران خدمات کارکنان هزینه شده است. همچنین پس از اجرای سناریوی تحقیق، محصول کل 95/0درصد افزایش یافته و رفاه خانوارهای شهری و روستایی به ترتیب 933/0درصد و89/0درصد و رفاه اجتماعی آن‌ها 93/0درصد و 87/0درصد بیش‌تر شده است. نتیجه‌گیری: بخش بهداشت و درمان دولتی به دلیل ماهیت آن، نقش اندکی در سایر بخش‌های اقتصادی دارد؛ با این حال، وجود تقاضای نهایی بالا و پرداخت‌ دستمزدهای کلان در این بخش سبب شده افزایش تقاضا تاثیرات گسترده‌ای بر تولید و رفاه اقتصادی داشته باشد؛ اما به علت بهبود وضعیت درآمدی پرسنل این بخش، که غالبا جزو دهک‌های بالای درآمدی جامعه هستند، توزیع درآمد در جامعه بدتر شده و لذا، دولت بایستی سیاست‌های حمایتی را جهت رفع این مشکل ارائه نماید. کلمات کلیدی: مخارج بهداشتی- درمانی دولت، ماتریس حسابداری اجتماعی، رفاه اجتماعی

Given the importance of the government involvement in the health sector, this study examined the economic and welfare implications of increasing the government healthcare expenditures in Iran. Methods: This applied research was conducted by utilizing the Social Accounting Matrix of the year 2011 prepared by the Iranian Parliament Research Center constructed based on the data such as the national accounts, the household budget survey of the year 2011, and updating of the I-O table of the year 2001. Estimates and data analysis were conducted using Excel 2013 software. Results: The results showed that most of the demand for the government health sector is in the form of final demand (99%) and most of the payments are paid to employees (67%). Moreover, after implementing the study scenario, the total output increased by 0.95% and the welfare of urban and rural households increased by 0.933% and 0.89% while the changes in their social welfare were 0.93% and 0.87%, respectively. Conclusions: Because of its nature, the government health sector has a slight direct role in other economic sectors. However, high final demand and huge wage payments in this sector have caused that the increasing demand creates widespread effects on the economic production and welfare. However, the improved income of employees in this sector, which are mostly included in the high-income deciles, will worsen the distribution of income to the detriment of the lower deciles. Therefore, the government should provide supportive policies to remove this problem.
