عنوان فارسی اجرای برنامه تحول نظام سلامت در دو محور «‌ترویج زایمان طبیعی و کاهش میزان پرداختی بیماران بستری دربیمارستان‌ها» در بیمارستان‌های تابعه دانشگاه علوم پزشکی همدان
چکیده فارسی مقاله مقدمه هزینه‌های فزاینده بهداشت و درمان، ظرفیت محدود نظام‌های تامین ‏مالی و ناکارآمدی شیوه‌های سنتی بخش سلامت، منجر به ‏سرمایه‌گذاری بیشتر دولت درحوزه سلامت و ایجاد اصلاحات لازم با اجرای برنامه‌های تحول نظام سلامت در وزارت بهداشت گردید. این مطالعه با هدف بررسی اثربخشی برنامه تحول نظام سلامت در دو محور «‏ترویج زایمان طبیعی و کاهش میزان پرداختی بیماران بستری دربیمارستان‌ها» در بیمارستان‌های تابعه دانشگاه علوم پزشکی همدان در سال 1393 انجام شد.‏ روش‌ کار در این مطالعه مقطعی که در سال 1394 انجام شد، اطلاعات مربوط به صورتحساب‌های کلیه بیماران بستری و آمار زایمان‌ها به تفکیک نوع زایمان در 15 بیمارستان دانشگاهی همدان، از طریق فرم‌های ثبت اطلاعات و سامانه وزارتی ‏جمع‌آوری و اثربخشی اجرای برنامه‌ها از دو طریق «مقایسه وضعیت قبل و بعد از اجرای طرح» و «‏مقایسه نتایج با معیارهای ابلاغی از سوی وزارت متبوع» بررسی گردید.‏ یافته ها یافته‌ها نشان داد در برنامه ترویج زایمان طبیعی و کاهش سزارین، آمار سزارین ‏پس از اجرای طرح 3/2% کاهش داشت. همچنین میزان پرداخت از جیب بیماران بستری در بیمارستان‌های دانشگاهی به 866/8% رسید. نتیجه گیری با اجرای طرح تحول نظام سلامت، آمار سزارین در بیمارستان‌های دانشگاه علوم پزشکی همدان کاهش داشته و می‌توان با تمهیداتی جدی، نسبت به بهبود بیشتر آن اقدام نمود. همچنین هزینه پرداختی بیماران بستری ‏دربیمارستان‌های دانشگاهی همدان، با توجه به دستورالعمل‌های وزارتی، کاهش بسیار چشمگیری داشته است و با رفع مشکلات  موجود در صورتحساب‌ها و نحوه محاسبه آن، می‌توان به تحقق کامل این برنامه رسید.

چکیده انگلیسی مقاله Introduction The increasing costs of health care, the limited capacity of financial systems, and the inefficiency of the traditional health sector practices led to more government investment in health and the necessary reforms through the implementation of health system development plans at the Ministry of Health. This study was conducted with the aim of evaluating the effectiveness of the health system's development plan in two axes: "Promoting natural vaginal delivery and reducing the payment rate of hospitalized patients in hospitals" in hospitals affiliated to Hamedan University of Medical Sciences in 2014. method In this cross-sectional study conducted in 1394, the information about the bills of all hospitalized patients and the statistics of deliveries by type of delivery in 15 universities of Hamedan from May 15 to March 4, 2014 through data registration forms and the Ministry of Health system and effectiveness Implementation of the programs was investigated through two methods: "Comparison of the situation before and after the implementation of the plan" and "Comparison of the results with the notification criteria by the Ministry". findings The results showed that in the program for the promotion of normal vaginal delivery and reduction of cesarean section, the cesarean section decreased by 3.2% after the implementation of the plan. Also, the rate of payment from the pockets of hospitalized patients in academic hospitals reached 866.8%. Conclusion With the implementation of health system reform, cesarean section has decreased in all hospitals of Hamedan University of Medical Sciences, except for Rifalfal Hospital, and it can be done with more serious measures. Also, the amount of payment from the pocket of the Hamedan university hospitals is acceptable in accordance with the ministry's instructions, and by solving the problems in the bills and how they are calculated, the full realization of this program can be achieved.
