عنوان فارسی بررسی فراوانی عوارض عضلانی اسکلتی در مبتلایان به دیابت مراجعه کننده به مرکز دیابت کاشان طی سال ۱۳۸۸
چکیده فارسی مقاله سابقه وهدف: دیابت شایع ترین بیماری متابولیک با تغییرات پاتوفیزیولوژیک در اندام ها است و عوارض آن هزینه زیادی به فرد، جامعه و سیستم بهداشتی تحمیل می کند. این تحقیق به منظور بررسی عوارض عضلانی اسکلتی در بیماران مبتلا به هر دو نوع دیابت صورت گرفته است. مواد و روش ها: برای انجام این مطالعه‌ی مقطعی سن، نوع دیابت و علائم بالینی درد مفصل و پارستزی و محدودیت حرکات مفاصل از شرح حال 327 بیمار دیابتی کسب شده و سپس توسط روماتولوژیست درگیری عضلانی-اسکلتی شامل کپسولیت چسبنده، انگشت ماشه ای، دوپویترن کنتراکچر، سندروم کانال کارپ و محدودیت حرکت مفاصل در آنها تشخیص داده شده و در پرسشنامه ثبت شدند. نتایج: میانگین سنی در بیماران نوع دو 61/12±6/54 و در بیماران نوع یک 36/10±7/29 سال بود. سندروم تونل کارپ در 8/49 درصد، استئوآرتریت زانو در 45 درصد، اسکلروداکتیلی در 2/27 درصد، دوپویترن کنتراکچر در 1/14 درصد، انگشت ماشه ای در 9/11 درصد، کپسولیت چسبنده در 9/11 درصد، سندروم محدودیت حرکت مفاصل در 8 درصد و مفصل شارکوت در 6/0 درصد افراد مشاهده شد. نوع دیابت با استئوآرتریت زانو (P< 0/003 OR=1/86 CI:1/05-3/30) و سندروم تونل کارپ (045/0>P) و طول مدت بیماری با انگشت ماشه ای، دوپویترن کنتراکچر (OR=3/76 CI:1/9-7/41) و محدودیت حرکت مفاصل رابطه معنی داری داشت. نتیجه گیری: یافته های این مطالعه نشان داد نوع دیابت و مدت ابتلاع بالای 15 سال در بیماران دیابتی منجر به افزایش بروز عوارض عضلانی اسکلتی می شود. لذا پیشنهاد می گردد به صورت دوره ای معاینه عضلانی اسکلتی در مبتلایان به دیابت انجام شود.

عنوان انگلیسی Frequency of musculoskeletal complications among the diabetic patients referred to Kashan diabetes center during 2009-10
چکیده انگلیسی مقاله Background: Diabetes Mellitus is one of the most common metabolic disorders causing pathophysiologic changes in multiple organs. Considering the burden of diabetes and its complications, this study aimed to evaluate the frequency of musculoskeletal complications in diabetic patients.Materials and Methods: This cross-sectional study was performed on 327 diabetic patients referred to Kashan diabetes center during 2009-10. Age, the type of diabetes and clinical manifestations such as arthralgia, paresthesia, and limited range of movement of joints were obtained from the hospital medical records of patients and then musculoskeletal complaints (e.g., adhesive capsulitis, carpal tunnel syndrome, dupuytren's contracture, trigger finger, and limited range of movement of joints) were diagnosed and recorded by a rheumatologist. Results: The mean ages of type II and I diabetic patients were 54.6±12.61 and 29.7±10.36 years, respectively. Carpal tunnel syndrome (49.8%), knee osteoarthritis (45%), sclerodactyly (27.2%), dupuytren's contracture (14.1%), trigger finger (11.9%), adhesive capsulitis (11.9%), limited joint movements' syndrome (8%) and Charcot joint (0.6%) were seen in patients. There was a significant relationship between the type of diabetes and knee osteoarthritis (P< 0.003 OR=1.86 CI=1.05-3.30) and also carpal tunnel syndrome (P< 0.045). A significant relationship was also seen between duration of disease and trigger finger, dupuytren's contracture (OR=3.76 CI=1.9-7.41) and limited joint movements. Conclusion: The findings of this study indicated that type of diabetes and also its duration (over 15 years) may increase the risk of musculoskeletal complications. Hence, careful periodic examinations of all diabetic patients are recommended.
