تأثیر مشاوره تلفنی بر تداوم و طول مدت شیردهی انحصاری زنان نخست‌زا |

زمینه و هدف: شیر مادر فواید سلامتی بی‌شماری برای مادر و نوزاد فراهم می‌کند. حمایت از شیردهی توسط کارکنان بهداشتی تأثیر مفید در طول مدت شیردهی به ویژه دوره تغذیه انحصاری با شیر مادر دارد. هدف از این مطالعه تعیین میزان تأثیر مشاوره تلفنی بر تداوم و طول مدت شیردهی انحصاری زنان نخست‌زا می‌باشد.روش بررسی: این کارآزمایی میدانی در مورد 140 نفر از زنان نخستزای مراجعه‌کننده به مراکز بهداشتی درمانی منتخب دانشگاه علوم پزشکی تهران در سال 1389 انجام یافته است. تعداد نمونه‌ها 70 نفر در گروه آزمون و 70 نفر در گروه شاهد بودند. گروه آزمون، مشاوره تلفنی را دریافت کردند در حالی که گروه شاهد تنها مراقبت‌های معمول را دریافت نمودند. روش گردآوری اطلاعات مصاحبه و ابزار گردآوری شامل پرسشنامه اطلاعات جمعیت‌شناختی و چک لیست بررسی تداوم و طول مدت شیردهی انحصاری بود. تداوم شیردهی انحصاری در دو گروه 1 و 3 ماه بعد از زایمان و طول مدت شیردهی انحصاری در هر دو گروه 3 ماه بعد از زایمان بررسی شد. تجزیه و تحلیل اطلاعات به کمک نرم‌افزار SPSS v.16 انجام یافت.یافته‌ها: میانگین طول مدت شیردهی انحصاری بر حسب هفته در گروه آزمون (99/3±05/8) به طور معناداری (008/0=p) بیشتر از گروه شاهد (03/4±23/6) بود. در ماه سوم پس از زایمان بعد از کنترل اثر متغیرهای مخدوش‌کننده، شانس شیردهی انحصاری در گروه آزمون به صورت معنادار بیشتر از گروه شاهد بود (03/0=p). همچنین انجام مشاوره تلفنی با طول مدت شیردهی انحصاری مادران ارتباط آماری معنادار داشت (02/0=p).نتیجه‌گیری: با توجه به تأثیر مشاوره تلفنی بر تداوم و طول مدت شیردهی انحصاری، ارایه این مشاوره که روشی ارزان و آسان است، می‌تواند به عنوان راه‌کاری جهت افزایش طول مدت شیردهی انحصاری به ویژه در مادران نخست‌زا مدنظر قرار گیرد. |

Effect of Telephone Counseling on Continuity and Duration of Breastfeeding among Primiparus Women |

Background & Aim: Breast milk provides many healthy nutrients to infants. Support provided by health care provider has positive effects on duration of breastfeeding. The purpose of this study was to determine the effects of telephone counseling on continuity and duration of breastfeeding among primiparus women. Methods & Materials: This clinical trial was conducted on 140 primiparus women referred to the selected health centers of Tehran University of Medical Sciences in 2010. The participants were randlomly allocated into two equal groups (each: 70 indviduals). The participants in the intervention group received telephone counseling by one of the researchers. The control group received routine care. Data were collected using a questionnaire including demographic characteristics and a breastfeeding check list. Exclusive breastfeeding rates were recorded one and three months after childbirth in both groups. Also, the continuity and duration of exclusive breastfeeding were recored three months after childbirth in both groups. Data were analyzed using the Chi-square test, Fisher&aposs exact test, standard logistic regression and linear regression in the SPSS-16 and the Win Pepi. Results: The average duration of exclusive breastfeeding in the intervention group (8.05±3.99) was significantly more than the control group (6.23±4.03) (P=0.008). In the third month of delivery, continuity of exclusive breastfeeding in the intervention group was more than the control group (P=0.03). Results also showed that telephone counseling was effective in the duration of exclusive breastfeeding (P=0.02). Conclusion: Telephone counseling was effective in continuity and duration of exclusive breastfeeding. Using this inexpensive and easy method can help to increase exclusive breastfeeding, especially in primiparus women. |
