عنوان فارسی گزارش یک مورد جراحی آندوسکوپیک موکورمایکوزیس بینی و سینوس پارانازال با حفظ عملکرد اعضاء حیاتی
چکیده فارسی مقاله چکیده زمینه و هدف: موکورمایکوزیس یک بیماری نادر و کشنده است که بر اثر قارچ‌هایی از خانواده موکوراسه ایجاد می‌شود. شایع‌ترین محل بروز آن بینی و مغز می‌باشد. هدف از گزارش این مورد تأکید بر تشخیص و درمان سریع برخی عفونت‌های فرصت‌طلب در بیماران دیابتی یا بیماران دچار ضعف سیستم ایمنی و نیز تأکید بر عمل جراحی با حفظ عملکرد اعضاء حیاتی و نقاط ضعف برخی آندوسکوپ‌ها در اعمال جراحی بینی است. شرح مورد: بیمار خانمی 76 ساله و دیابتی که به علت درد شدید سر و صورت و کاهش شنوایی با تشخیص موکورمایکوزیس سینوس‌های پارانازال در بیمارستان مرادی شهرستان رفسنجان بستری گردید و پس از دو مرحله عمل جراحی (آندوسکوپی سینوس و مدیال ماگزیلکتومی) و درمان طبی با آمفوتریسین B پس از 45 روز بستری، بهبودی یافت. بحث و نتیجه‌گیری: در بیمارانی که با سردرد یا درد ناحیه صورت مراجعه می‌کنند معاینه دقیق و گرفتن شرح حال از نظر وجود بیماری‌های زمینه‌ای و مصرف داروهای سرکوب‌کننده سیستم ایمنی، حفظ اعضاء حیاتی ضمن عمل جراحی جهت برداشتن بافت‌های درگیر و انجام آندوسکوپی به روش کالدول لوک ( Caldwell-luc ) جهت دسترسی بهتر به دیواره قدامی سینوس ماگزیلا باید مد نظر قرار گیرد. واژه‌های کلیدی: موکورمایکوزیس، سینوزیت، جراحی آندوسکوپی

Mokhtaree [4] Received: 25/09/07 Sent for Revision: 15/09/08 Received Revised Manuscript: 30/04/09 Accepted: 09/05/09 Background and Objectives: Mucormycosis is a rare but often fatal disease caused by a fungus from the group of Mucorales. It most commonly affects the nose and brain. The aim of this report is the emphasis to quick recognition and treatment of some opportunistic infections in patients with diabetes or suppressed immune systems and emphasis on surgery with protection of vital organs functions and limitations of some endoscopes in the nasal surgery. Case Report: This report presents a 76-year old diabetic female with severe headache, facial pain and hearing loss and recognition of paranasal sinus mucormycosis who was hospitalized in Rafsanjan Moradi Hospital. After two successive surgeries, (endoscopic surgery and medial maxillectomy) and medical therapy with Amphotricin B after 45 days was recovered. Conclusion: In the patients who are referred with headache or facial pain, exact examination and history-taking about underlying disease and immunosuppressive drugs is necessary. Protection of vital organs in the surgery for removeal of necrotic and infected tissues and endoscopic surgery with Caldwell-Luc procedure for better access to anterior wall of maxillary sinus must be considered.
