عنوان فارسی مقایسه تأثیر AFO مفصل‌دار و ثابت بر عملکرد ایستادن و راه رفتن کودکان فلج مغزی اسپاستیک ۸-۴ سال مراجعه‌کننده به مؤسسه توانبخشی ولیعصر (عج) شهر تهران
چکیده فارسی مقاله چکیده زمینه و هدف: ایستادن و راه رفتن از بیشترین فعالیت‌هایی هستند که انسان به صورت روزمره انجام می‌دهد. در کودکان فلج مغزی به دلیل آسیب سیستم اعصاب مرکزی، اختلالاتی در این عملکردها به وجود می‌آید. یکی از راه‌های بهبود عملکرد ایستادن و راه رفتن آنها، استفاده از AFO ( Ankle foot ortosis ) می‌باشد. از این‌رو، این مطالعه به منظور مقایسه تأثیر دو نوع AFO مفصل‌دار و ثابت بر عملکرد ایستادن و راه رفتن کودکان فلج مغزی اسپاستیک انجام شد. مواد و روش‌ها: مطالعه به صورت شبه تجربی مداخله‌ای، بر روی30 کودک فلج مغزی اسپاستیک 8-4 سال مراجعه‌کننده به مؤسسه توانبخشی ولیعصر (عج) شهر تهران انجام شد. این کودکان به طور تصادفی به سه گروه 10 نفره دسته‌بندی شدند. برای گروه‌های اول و دوم به مدت 3 ماه AFO مفصل‌دار یا ثابت تخصص یافت. گروه سوم بدون AFO بود. عملکرد ایستادن و راه رفتن به وسیله آزمون عملکرد حرکتی درشت-88 ارزیابی شد. یافته‌ها : در هر سه گروه، بعد از مداخله در عملکرد ایستادن و راه رفتن اختلاف معنی‌داری دیده شد (005/0= p ). اختلاف عملکرد سه گروه از نظر میانگین کلی امتیازهای قبل و بعد از مداخله معنی‌دار بود (015/0 p= ). مقایسه عملکرد گروه مفصل‌دار، با گروه ثابت و گروه شاهد تفاوت معنی‌داری را نشان داد (000/0= p ). اما بین دو گروه ثابت و شاهد تفاوتی دیده نشد. نتیجه‌گیری: استفاده از AFO مفصل‌دار تأثیر بیشتری بر بهبود عملکرد ایستادن و راه رفتن کودکان فلج مغزی اسپاستیک دارد و می‌توان آن‌ را به کودکان فلج مغزی که با و یا بدون وسیله کمکی می‌ایستند و یا راه می‌روند توصیه نمود. واژه‌های کلیدی: AFO مفصل‌دار و ثابت، فلج مغزی، ایستادن و راه رفتن

In children with cerebral palsy (CP), lesion in central nervous system could cause impairment in these functions. One way to improve their standing and walking functions is the use of Ankle Foot Orthoses (AFOs). This study was performed to compare the effects of hinged and solid AFOs on standing and walking functions in 4-8 year old children with CP spastic. Materials and Methods: This quasi-experimental and interventional study was performed on 30 children (4-8 years old) with CP spastic in vali-Asr rehabilitation foundation of Tehran. Children were divided into 3 groups (of ten children each group) randomly groupe 1 received hinged AFO, groupe 2 solid AFO, and group 3 (control group) received no AFOs for 3 months. Standing and walking functions were evaluated by Gross Motor Function Measure-88. Results: There were significant differences in standing and walking functions after intervention in the 3 groups (p=0.005). There were also significant differences between the 3 groups in terms of mean differences of total scores before and after the intervention (p= 0.015). There were significant differences between the two groups hinged AFO and solid AFO and between hinged AFO group and control group, (p= 0.000), but between the two groups, solid AFO and control, there was no differences. Conclusions: Hinged AFO showed more improvement in standing and walking functions among children with ‍ CP spastic, and could be recommended for children with ‍ CP, especially, for those who could stand up or walk with or without assistive devices.
